Prerequisite
One of the main goals of Population Health Management is improving health across a broadly defined continuum. How have approaches to care coordination and management evolved over the years and what have we learned to better understand and effectively manage population health? What approaches have been used to transition from a health-system centric model of clinical care to model where health management with both clinical and non-clinical partnerships is the focus? This course helps to answer these and many other questions related to managing the health of populations.
Performs in-depth explorations of a variety of contemporary models for care coordination and management. Encourages students to evaluate different strategies for identifying modifiable risks, aligning appropriate services, and integrating non-clinical partners in improving the health of populations. Introduces the newest standards in the population health management category of Health Plan Accreditation.
Learning Objectives
Upon successfully completing this course, students will be able to:
- Provide examples of modern approaches to care coordination that extend the continuum beyond the healthcare system and into the community
- Describe the role of non-clinical partners in addressing the needs of vulnerable populations and underserved communities
- Evaluate contemporary care management models and their strategies for identifying populations with modifiable risks, aligning services to the needs of the population, and integrating the various services
- Interpret data from a variety of sources to identify populations of interest and to recommend appropriate community-oriented care pathways
- Develop strategies for meeting standards in NCQA’s (National Committee for Quality Assurance) new population health management category of Health Plan Accreditation
